Nativity Sign Up Form
Please fill out the following “Sign Up Form” for each sport your child would like to participate in. Don’t forget your check for the corresponding fee (as seen on the form) of the sport your child wishes to play.
Remember to Register at the CYO website and turn in your mandatory 2011-2012 school year CYO Physical Form to a coach or a Nativity Athletics Committee member.
